What to bring to your appointment

Bringing the right records can help your appointment move more smoothly.

  • Medical Form I-693 (Fill page 1 and the top of page 2, in CAPITAL letters)

  • Passport or Driver's license as picture ID

  • Vaccination records (if available)

  • Alien Registration Number (A Number, if available)

Before you arrive

  • Do not sign Form I-693 before your appointment.

  • Tell us when you schedule if you need language assistance.

  • Call if your records are incomplete or in another language.

  • Ask what the $405 fee includes and whether any additional services may be needed.

Medical requirements vary. Required tests and vaccines can depend on age, medical history, prior records, and current federal guidance. The civil surgeon will review what applies to you.

Keep your USCIS paperwork protected

Follow the civil surgeon's instructions for your completed Form I-693. USCIS generally requires the completed form and supporting documents to be handled according to its current filing instructions. Do not open a sealed envelope unless the current USCIS filing instructions specifically direct you to do so for online filing.
